Skyline Ridge Apartments and Nursing & Rehabilitation Center is a beautiful community in Canon City, CO, with more than 80 units offering skilled nursing care.

Key Responsibilities

At this time, we are seeking a full time (40 hours/week) Admissions Director to join our team! Schedule work will be Mon – Fri. The wage range for this position is from $19.00- $19.57hour, commensurate with experience.

FUNCTION: Is responsible for planning and coordinating admission to the facility. The admission director seeks and/or receives inquires and documents information for potential residents to be admitted.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential function satisfactorily. The requirements listed below are representative of the knowledge and skill and/or ability required.

Minimum Qualifications:

  • Freedom from illegal use of drugs
  • Freedom from use of drugs and alcohol in the workplace
  • Anyone found guilty by a court of law of abusing, neglecting or mistreating individuals in a health care or related setting is ineligible for employment in the position.
  • At least one-year previous experience in the health care field, preferably long-term care.
  • Desire to market/knowledge of area markets
  • Familiarity with medical terminology
  • Skilled generalist in patient assessment.
  • Ability to work under pressure
  • Excellent interpersonal skills
  • Ability to work in a team setting
  • Valid State Drivers License

Education and Experience:

  • High school diploma or equivalent
  • Degree and/or certification in health care or marketing preferred.

Certificates, Licenses, Registration: None required.

Language Skills: Must be able to effectively express self in written and oral communication and to communicate with a wide variety of health care professionals, potential residents, family members and the community in general. Must have the ability to develop a presentation and speak in front of both large and small public groups.

Mathematical Skills: Ability to determine Medicare and Medicaid Status, and to calculate level of care.

Reasoning Ability: Ability to define and solve problems collects and interprets information, establish facts, and draw valid conclusions. Ability to understand federal and state regulations and to deal with several abstract and concrete variables.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Develop and implement a marketing program for the facility in coordination with the administrator and care team.
  • Information is received and documented on all new resident referrals and necessary data for completion of all phases of assessment and admission are completed.
  • Screening evaluations are performed on each referral, and criteria to determine whether the prospective resident meets criteria for transfer and admission are objectively applied.
  • Verify financial support.
  • Referring and receiving physician communication is coordinated and facilitated.
  • Communication with the referral source, resident, family (responsible party) insurance representative, and the corporate office related to the status of referrals, progress, and discharge plan is maintained.
  • Documentation on all admissions and discharge activity is maintained.
  • An outcome data tracking system is developed and maintained.
  • Acts as a liaison between Facility and hospital Social Workers/Discharge Planners
  • Advises residents and/or families of services available at the facility
  • Provides written information about the services offered
  • When in the facility, takes all phone calls related to admissions
  • Provides tours of the facility to prospective residents and/or families
  • Works on census development through communication with hospital discharge planners, community home health agencies, hospice organizations, Physician’s offices etc.
  • Current knowledge of state and regulatory requirement related to admissions, discharge and transfer is demonstrated.
  • Current knowledge and understanding of payment systems, HMO, PPO, Indemnity, Worker’s Compensation, insurance (Medicare and Medicaid, etc.) is demonstrated.
  • May be responsible for customer satisfaction surveys, marketing surveys etc.
  • Coordinates and schedules pre-admission conferences
  • In coordination with the full admissions team, registers resident, and provides all essential information to the resident and family. Insure that the appropriate signatures are obtained to the facility to provide care.

INTERPERSONAL SKILLS: Demonstrates active listening skills; maintains effective relationship with customers.

CONTINUING EDUCATION: Attends in-service and education programs, as required for any professional certification or licensure associated with the position, and to maintain up to date knowledge of admission process requirements.

PHYSICAL DEMANDS:

  • Sufficient mobility and strength to move freely through the building to assure resident safety at all times, and to assist, transfer or otherwise move residents of the facility out of danger in case of an emergency.
  • Ability to stand and walk; use hands to finger, handle or feel; reach with hands and arms, talk and hear. Occasional physical effort with light to medium objects.
  • Able to carry out essential functions of this job (with or without reasonable accommodation) without posting specific, current risk of substantial harm to health and safety of others.
  • Must be able to cope with the mental and emotional stress of the position
  • Must be able to see and hear, or use prosthetics that will enable the senses to function adequately to ensure that the requirements of the position are fully met.

WORK ENVIRONMENT:

  • Office and field, including hospitals, rehabilitation setting, other nursing homes, and private homes.
  • May be required to wear facemask, gown or gloves.
  • Works beyond normal worker hours when necessary

COMMUNICATION: Must be able to demonstrate excellent communications skills with a wide variety of individuals, including but not limited to families, residents, discharge planners, and trustees.
