Our Corporate office is located in Newton, Massachusetts. Founded in 2000, Five Star has two major operating divisions: Five Star Senior Living and Five Star Rehabilitation Services.

This position is REMOTE, coming to the Corporate office in Newton, MA once a month.

SUMMARY: The National Director of Clinical Operations oversees the Ageility clinical management team. They are responsible for developing and implementing programs and policies for the Ageility. This position will oversee and educate the field on regulatory changes specific to Rehabilitation.

REQUIREMENTS:
Minimum Qualifications:

  • Graduate of an accredited physical therapy or occupational therapy College. Five years of clinical and operational experience required. Experience in Senior Living and Outpatient preferred.
Education and/or Experience:
Graduate of an accredited bachelor’s, master’s or Doctorate level physical therapy or occupational therapy College.
Certificates, Licenses, Registrations:
Licensed by the state in which the therapist resides or practices.
Active, Valid Driver’s License and able to travel between facilities.

RESPONSIBILITIES:
Rehab/Clinical Responsibilities
  1. Monitoring and tracking of clinical programs.
  2. Establishes proven outcome measures to benchmark clinical efficacy in outpatient industry and across Divisions and Regions.
  3. Implements and monitors EHR and clinical indicators in Rehab software platform.
  4. Collaborates with Learning and development to roll out and monitor training (CEU programs).
  5. Establishes and oversee SOX regulations related to clinical services.
  6. Collaborates with Divisional operators to understand and support their clinical programming and outcome needs.
  7. Develops and updates all Rehab clinical policies and procedures.
  8. Updates clinical area on Sharepoint.
  9. Successful implementation and monitoring of - clinical programs, processes and policies for onboarding and orientation.
  10. Establishes and monitors competencies for Ageility Team Members.
  11. Provides comprehensive reporting such as outcomes and program analysis for Quality of Care committee on a quarterly basis.
  12. Collaborates with VP of Ageility on outcomes and programs to establish the Ageility clinical differentiators for outpatient programs.
  13. Works collaboratively with Director of Fitness to establish to enhance services throughout the continuum in senior living.
  14. Works collaboratively with Home Health Administrator to enhance initiatives in the post-acute continuum of services.
  15. Facilitates and supports enhancement of Mentor program to support the field in operations, startups and survey preparation.
  16. Responsible for state and AAAASF surveys with focus on survey preparedness, timely applications and plans of correction post survey.
  17. Supports the education of clinical programs in startups including documentation training, implementation of programs and EHR.
  18. Manages the reporting of client incidents including monthly reviews and enhancement of database to identify trends and educational opportunities.
Regulatory Compliance Responsibilities: Gathers, analyzes and identifies educational opportunities based on quarterly chart audit roll ups.
  1. Development training and competencies for Rehab portion of ICD-10.
  2. Stays current with Medicare changes and updates and provides training to the staff accordingly.
  3. Oversees Medical Reviewer to identify Rehab trends and learning opportunities related to Medicare.
  4. Oversight of Clinical Specialists, Inpatient Specialist, Medical Record Reviewer and Mentors.
  5. Analyzes appeal trends and provides education accordingly.
  6. Works with Medical Reviewer on ALJ hearings.
  7. Rehab HIPPA oversight.

Our specialty programs includes our Step up to Stop falls Program, "Connections" our memory care therapy program, and our executive functioning program that focuses on cognitive strategies aimed at keeping our residents independent in areas such as medication management and house hold functions.
